Endosseous implants have been successfully used to restore edentulous mandibles with implant supported fixed bridges, hybrid prosthetic dentures and removable overdenture prostheses. However, atrophy of edentulous ridges may limit implant placement in the mandible. Anatomic limitations and resorbed alveolar ridges may compromise implant number, length, and inclination. The use of traditional implants sometimes requires extensive surgery, ridge augmentation, or bone grafting.

Small diameter implants placed with flapless surgery to support pre-existing conventional dentures present an alternative method of restoring patients with atrophic mandibles. I have had the opportunity to use the new 2.1mm diameter mini dental implants. They are a great addition to the mini implant line from IMTEC, the global leader. Fortunately the same pilot drill and insertion instruments are utilized to place the 2.1mm diameter implants. The added bulk in diameter seems to enhance fracture toughness, surface area, and strength. The addition of the micro thread appears to maintain cortical bone at the neck of the implant. Most importantly, the new size and design enhance implant to bone contact, which is essential for immediate load. Also, I have found that the 10, 13, and 15mm lengths are the most used. There are very few occasions where I have used the 18mm length.
