Step-By-Step: Replacing a Worn Posterior Amalgam with IPS Empress Direct

A patient presented with a worn amalgam on tooth #14. Here's a pictorial of how we restored this tooth with IPS Empress Direct restorative from Ivoclar Vivadent. 

1. Here's the pre-op situation. Notice the necrotic gingival tissue.

 

2. The first step was to select a shade using the shade guide supplied with the IPS Empress kit.

 

3. The old amalgam as well as any residual caries were removed.

 

4. The etching step involved treating enamel for 10 seconds...

 

5. ...and then the dentin for 10 seconds.

 

6. After applying AdHese with the VivaPen applicator system and light curing, Tetric Flow was placed on the floor of the prep.

 

7. Then IPS Empress Direct dentin shade A1 is placed into the prep, just short of  the enamel layer.

 

8. Next, IPS Empress Direct enamel shade A1 is placed to the cavosurface margins.

 

9. To create lifelike anatomy and characterization, Tetric shadesochre and transparent 30 were placed to accent occlusal grooves. Then the restoration was polished with AstroPol cups and points.

 

10. Remember the pre-op condition. Here's the result.
